Thank you for being a Special Olympics Massachusetts Class A Volunteer. Please complete the 4 sections of this application form. Upon submittal, you will be directed to a secure form to enter in your Social Security Number which is required for both a national background check and MA CORI check, as well as Drivers License information only if your volunteer role involves transporting Special Olympics athletes.
You will be emailed a PDF of the MA CORI check form that requires a handwritten signature and the form to be verified in person by a Special Olympics MA CORI authorized representative. Please print the PDF, sign and date it, and return it to Special Olympics (more detailed instructions will be included in the confirmation email).
